What Is Premature Ejaculation?
Premature ejaculation (PE) occurs when a man ejaculates too soon during sexual activity, ruining sexual experiences for both partners.
PE is relatively common. Approximately 30% of men experience it at some point in their lives. PE may be caused by psychological factors (such as stress or anxiety) or physical factors (including prostate conditions or hormonal imbalances). In some cases, it may occur as a side effect of certain medications.
Occasional episodes of premature ejaculation should not be a cause for concern. However, if PE occurs regularly and interferes with sexual satisfaction, treatment may be appropriate.
Treatments for PE
Treatments for premature ejaculation include behavioral techniques, condoms, topical anesthetics, and oral medications.
Many men benefit from combining two or more of these approaches for the best overall results. At Careborne Sensible Medical, we prescribe medications to treat premature ejaculation.
Topical numbing agents
Lidocaine-prilocaine (Lidopril, EMLA Cream) 2.5%/2.5%
Available as cream or gel, these topicals contain two anesthetics - lidocaine and prilocaine. The medication is applied 10-15 minutes before sexual activity to reduce penile sensitivity and help delay ejaculation.
Oral medications
Many medications - including certain antidepressants, pain relievers, and phosphodiesterase-5 (PDE-5) inhibitors - may be prescribed off-label to help delay ejaculation.
Antidepressants
Selective serotonin reuptake inhibitors (SSRIs) include paroxetine (Paxil®, Pexeva®, Brisdelle®), escitalopram (Lexapro®), citalopram (Celexa®), sertraline (Zoloft®), and fluoxetine (Prozac®). These medications may be taken daily or on an as-needed basis before sexual activity.
If SSRIs are not effective or not well tolerated, a second-line option may be the tricyclic antidepressant clomipramine (Anafranil®). During the consultation, we will determine which option is appropriate for you.
Pain Relievers
Tramadol (Ultram®) has some SSRI activity and may help delay ejaculation. However, because it is a controlled medication, its use for premature ejaculation is limited.
Phosphodiesterase-5 (PDE-5) Inhibitors
Medications used to treat erectile dysfunction may also help with premature ejaculation. These include sildenafil (Viagra®), tadalafil (Cialis®), avanafil (Stendra®), and vardenafil. PDE-5 Inhibitors can generally be used in combination with other treatments.

You need to know your average blood pressure and have recent labs handy. Many local pharmacies have machines that can measure your blood pressure for free. Labs can be ordered through www.ultalabtests.com or any other online platform.
Medications used to treat premature ejaculation are generally well tolerated and typically cause only mild side effects. However, if you take nitrates or certain heart medications, some of these treatments may lower blood pressure. Therefore, it is important to provide a complete medication history during your consultation.
