Privacy Matters
This Website Privacy Notice (WPN) applies to personal data collected and processed online by Careborne Sensible Medical by Salud Medical and Consulting LLC (SMC or WE/US/OUR) and its subsidiaries and affiliated companies, including the medical practices and other healthcare entities with which SMC contracts. WPN applies to all SMC websites (SITES), including www.careborne.com, applications, and services. WPN does not apply to websites, applications or services that display or link to different privacy notices.
By using SITES, you are accepting the practices described in this WPN. SMC will not sell, distribute, or lease your personal information to third parties unless WE have your permission or are required by law to do so.
Types of Information Gathered
You provide personal information when you search, read, and interact with SITES, buy a service and/or communicate with US. As a result of those actions, you supply US with the information as described below.
Information You Give Us. WE receive and store any information you enter on SITES, provide by using OUR services, or give US in any other way, such as, your first and last name, email address, phone number, billing/service address, date of birth, etc. You can choose not to provide certain information, but then you might not be able to use OUR services.
Automatic Information. WE are able to obtain certain types of anonymous information which is not personally identifiable when your web browser accesses SITES. Some of the information WE may collect and analyze includes the Internet protocol (IP) address, viewing patterns, browser software and version, operating system, search engine keywords, and any additional information relating to you.
Cookies. Your behavior on SITES may be tracked and logged using cookies in order to analyze aggregated user behavior for the purpose of improving the effectiveness of SITES. Anonymous Information. SMC uses the anonymous browsing information collected automatically by OUR servers primarily to help US administer and improve SITES. WE may also use aggregated anonymous information to provide information about SITES to potential business partners and other unaffiliated entities. This information is not personally identifiable.
Information from Other Sources. WE might receive information about you from other sources and add it to OUR account information.
Mobile. When you access SITES from your mobile device, download or use apps, WE may receive information about your location and your mobile device, including a unique identifier for your device. WE may use this information to provide you with location-based services, such as advertising, search results, and other personalized content. Most mobile devices allow you to turn off location services.
Payment information. SMC reserves the right to contract any payment platform available in the market, which treats your data for the exclusive purpose of processing the purchase of OUR services. Your credit card information or payment information will be processed, based on your choice, by Google Pay, PayPal, Stripe, or another payment platform. Google Analytics. WE use Google Analytics provided by Google, Inc., USA (“Google”) to enhance SITES and improve services. These tool and technologies collect and analyze certain types of information, including IP addresses, device, and software identifiers, referring and exit URLs, feature use metrics and statistics, usage and purchase history, media access control address (MAC Address), mobile unique device identifiers, and other similar information via the use of cookies. The information generated by Google Analytics (including your IP address) may be transmitted to and stored by Google on servers in the United States. Communication with You
WE communicate with you via email, phone, and text. WE also use your email address to send you notice of payments, to send information about important changes to OUR services, promotional messages about new services, and to send notices and other disclosures required by law. Also, to help US make emails more useful, WE may receive a confirmation when you open or receive email from US. You agree to receive such communications electronically. If you do not want to receive promotional messages or other communication from US, please send US a written request. Editing & Deleting Information
You can change, modify, rectify, and delete your information at any time. If you believe that any information, WE hold about you is incorrect or incomplete, please contact US via SITES as soon as possible. WE will promptly correct any information found to be incorrect.
How Information Shared
WE do not sell your information to others. WE do share customer information only as described below.
Third-Party Advertisers & Links to Other Websites. SITES may display targeted advertising and links to other websites based on your personal preferences. WE do not provide any personally identifiable user information to these advertisers or third-party websites. However, these third-party websites and advertisers sometimes use technology to send (or "serve") the advertisements that appear on SITES directly to your browser. They automatically receive your IP address when this happens. They may also use cookies, JavaScript, web beacons (also known as action tags or single-pixel gifs), and other technologies to measure the effectiveness of their ads and to personalize advertising content. WE do not have access to or control over cookies or other features that they may use, and the information practices of these advertisers and third-party websites are not covered by WPN. Social Networks. You will find links to different social networks on SITES, through which you can share your information. It is advisable to consult the privacy policy and data protection of each social network.
Business Transfers. If SMC creates, merges with, or is acquired by another entity, your information will most likely be transferred. WE will email you or place a prominent notice on SITES before your information becomes subject to another privacy policy.
Protection of SMC & Others. WE release personal information when WE believe release is appropriate to comply with the law, enforce OUR policies, or protect the rights, property, or safety of SMC, OUR users, employees, contractors, or others. This includes exchanging information with other companies and organizations for fraud protection and credit risk reduction.
With Your Consent. Other than as set out above, you will receive notice when personally identifiable information about you might go to third parties, and you will have an opportunity to choose not to share the information.
Protecting Information
SMC follows generally accepted industry standards to protect your personal information, both during transmission and once SMC receives it. WE work to protect the security of your information during transmission by using Secure Sockets Layer (SSL) software, which encrypts information you input. If transactions are processed on SITES, transaction information is transmitted in encrypted form using industry-standard SSL connections. WE restrict authorized access to your personal information to those persons who have a legitimate purpose to know that information to provide services to you and those persons you have authorized to have access to such information.
Notwithstanding the above, no method of transmission over the Internet, or method of electronic storage, is 100% secure. Therefore, while SMC strives to use commercially acceptable means to protect your personal information, WE cannot guarantee its absolute security.
Your Rights
You have the following rights in relation to your personal data provided to SMC:

If you consider that the processing of your personal data is not appropriate to the regulations, as well as in the case of not seeing the exercise of your rights satisfied, the control authority in which the claim was filed will inform the claimant about the course and result of the claim.
Children’s Online Privacy Protection
WE are in compliance regarding the protection of the personal data of minors. WE do not collect any information from anyone under 13 years of age. SITES and services are all directed to people who are at least 18 years old or older. If you are under 18, you may use SITES and services only with the involvement of a parent or guardian.
Suppliers & Other Third Parties
Except as otherwise expressly included here, this Notice addresses only the use and disclosure of information SMC collects from you. If you disclose your information to others, including OUR contractors, different rules may apply to their use or disclosure of the information. SMC does not control the privacy policies of third parties, and you are subject to the privacy policies of those third parties where applicable. WE are not responsible for the privacy or security practices of other websites on the Internet, even those linked to or from SITES. SMC encourages you to ask questions before you disclose your personal information to others.
Changes to This Notice
Current WPN is posted on SITES. SMC reserves the right to amend or modify this WPN at any time. If WE make any changes to WPN, WE will change the updated date above. Your continued use of SITES after We make changes is deemed to be acceptance of those changes, so please check this policy periodically for updates. Any information collected under this current WPN will remain bound by the terms of WPN. After the changes take effect, all new information collected, if any, will be subject to the revised WPN.
