Payment, Insurance & Registration
Payment is non-refundable. Your payment, which may appear as “Consultation” or “Elias Lu” on your receipt or statement, is non-refundable regardless of the outcome of the consultation, treatment, or service, or your level of satisfaction with the care provided. Payment reserves the professional time and services associated with your appointment and is not contingent upon a particular diagnosis, treatment recommendation, prescription, or other outcome.
Insurance
Careborne is a self-pay, out-of-network practice. I do not bill insurance and do not perform or assist with insurance preauthorization, coverage verification, claims, or reimbursement requests. If you intend to seek reimbursement from your insurance company, you are responsible for determining whether your plan provides any out-of-network benefits and for submitting any documentation required by your insurer.

Please complete the registration form below as thoroughly as you can. The more I know about your health, the better prepared I'll be before we meet, which means we can spend less time playing scribe and more time helping you feel better.
That said, don't overthink it. This isn't a college entrance exam, and there are no trick questions. If you're not sure about something, take your best guess – we’ll sort out the details during your visit. Keep moving!
