Last revised: October 2026
This document sets out the policies of our practice and the terms under which care is provided. Please read it carefully before scheduling or paying for a visit. If any part is unclear, contact us before you book, and we will address your questions.

In this document, “we,” “us,” “our,” and “the Practice” refer to the practice through which you schedule services, including Careborne Sensible Medical. “You” refers to the patient and, where applicable, the patient’s parent, legal guardian, or authorized representative.

This summary is provided for convenience. The complete terms that follow govern.

-
No guarantees. We provide care to the best of our professional ability but cannot guarantee any particular outcome or the issuance of a prescription.
-
No refunds. Visit fees are non-refundable once a visit has taken place, including when no prescription is issued or you are dissatisfied with the outcome.

24-hour cancellation policy. Appointments canceled with less than 24 hours’ notice, or missed entirely, are charged the full visit fee.
-
Prescriptions. Prescriptions are sent electronically to the pharmacy you select and cannot be edited or redirected once transmitted.
-
Refills. Refills and medication changes require a new visit, except for one same-condition adjustment within seven days of the original visit, at our discretion.

Telemedicine has limitations. It does not permit a hands-on examination or in-office testing, and it is not appropriate for emergencies.
-
House calls. Care outside the office is subject to practical limitations and a release of liability.
-
Emergencies. In an emergency, call 911. Do not wait for a response from us.

1. Practice Overview
Scope of Care
We evaluate and treat routine, non-life-threatening health concerns in person, at your home by arrangement, and by secure video, audio, or messaging. At each visit, we review your symptoms and history, discuss our clinical assessment, and develop a plan of care with you.

Our services may include the following. Availability depends on the type of visit (in person, telemedicine, or house call) and on your location.

-
General care – wellness examinations, preventive screening, and management of chronic conditions
-
Diagnostics – physical examination and laboratory testing of blood, urine, saliva, stool, or tissue samples
-
Prescriptions and recommendations – pharmaceuticals, supplements, vitamins, botanicals, minerals, and topical preparations
-
Acupuncture and related therapies – dry needling, electrostimulation, infrared and laser therapy, ultrasound, manual therapy, massage, cupping, and point bleeding
-
In-office procedures – joint aspiration and injection, ear irrigation, cryotherapy, abscess drainage, suturing of minor lacerations, and wound care
-
Injections and administered medications – intramuscular, subcutaneous, and intradermal injections; mesotherapy; trigger point and acupuncture point injections; medication electrophoresis; nebulizer treatments; and intranasal medications
-
Referrals – to imaging, laboratory, or specialist services when indicated
-
Documentation – work and school notes, disability evaluations, emotional support animal (ESA) letters, EMC determinations, and other medico-legal documentation, issued at the provider’s clinical discretion and in accordance with applicable law
Scope of Practice
We are a limited-scope practice. Unless agreed beforehand, we will not serve as your primary care provider. We are not equipped to provide emergency, urgent, or inpatient care or to manage psychiatric emergencies. Our services are intended to complement, not replace, the care you receive from other health care providers. We cannot guarantee the availability of any particular service in the future.

You are expected to inform your primary care provider of the care you receive from us. If your condition does not improve or worsens, you are responsible for seeking further evaluation from a qualified health care provider.

What We Can and Cannot Promise
We cannot predict every outcome or guarantee that treatment will resolve your condition. Treatment may be ineffective, and symptoms may occasionally worsen before they improve. If this occurs:

-
Contact your primary care provider first, if available.
-
If you cannot reach your primary care provider, go to an urgent care center.
-
In an emergency, call 911 or go to the nearest emergency room. Do not wait for a response from us.
Please notify us promptly of any unexpected or concerning symptoms during or after treatment.

Emergencies During a Visit
If a medical emergency occurs during a visit, we will contact 911. You are responsible for all resulting costs, including emergency transport and hospital care. For telemedicine visits, you must provide your current physical address at the start of each visit so that emergency services can be dispatched to you.

Right to Decline or Discontinue Services
You may decline any treatment or test at any time, before or during a visit. We may decline or discontinue services for the reasons described in Section 10 or where we determine, in our professional judgment, that continued care is unsafe or inappropriate. Where clinically appropriate, we will provide reasonable notice, assist with the transition of your care, and make your records available to you or your new provider upon request.

2. Communication
Contacting Us
The preferred methods of contact are the patient portal and email. We respond to non-urgent messages within one business day. For urgent, non-emergency matters, call or text your provider.

Our messaging channels are not monitored continuously and must not be used for emergencies. In a life-threatening emergency, call 911, including outside our office hours. If you are experiencing a mental health crisis or thoughts of self-harm, call or text 988 (the 988 Suicide & Crisis Lifeline).

Scheduling Conflicts
If an emergency or scheduling conflict prevents you from attending a visit, contact us as soon as possible. We will make reasonable efforts to reschedule, subject to availability. Changes requested with less than 24 hours’ notice are subject to the cancellation policy in Section 4, and any waiver of the cancellation fee is at our sole discretion.

3. Privacy and Confidentiality

Privacy Practices
We take the privacy of your health information seriously. Our Notice of Privacy Practices, which describes how we collect, use, and protect your health information and your rights of access to your records, is posted on our websites.

Communication Channels
Video visits are conducted through a secure, encrypted platform. We may also contact you by standard text message, email, or telephone for scheduling and follow-up. These channels are not fully secure, and by using them you acknowledge and accept the associated risk. If privacy is a primary concern, please use the patient portal.

Any notice or message sent to the contact information you provided at registration is considered received upon delivery, whether or not you have opened it. You are responsible for keeping your contact information current.

Recordings and Images
Telephone calls, emails, messages, photographs, and virtual visits may be recorded and retained as part of your medical record. We may ask to photograph a limited area of your skin or body for diagnosis, documentation, and consultation with other health care providers; you may decline. Photographs are maintained in your medical record and disclosed only as permitted by law.
We use industry-standard safeguards; however, no system is entirely secure. By using our services, you accept this residual risk.

4. Fees and Payment

Self-Pay Practice
We are a self-pay practice. We do not bill insurance, Medicare, or Medicaid, and we do not submit claims or obtain prior authorizations; any such process is solely between you and your health plan. You are responsible for the full cost of your care regardless of your coverage. Upon request, we will provide an itemized receipt that you may submit to your insurer, although we cannot guarantee reimbursement.

Fees
Fees are charged in accordance with our published Fee Schedule, with payment due as indicated at the time of scheduling. We reserve the right to change our fees at any time; changes do not apply to visits already paid for.

Refunds and Cancellations
Visit fees are non-refundable once a visit has taken place, including when no prescription is issued or you are dissatisfied with the outcome. Appointments canceled with less than 24 hours’ notice, or missed entirely, are charged the full visit fee.

5. Prescriptions and Medications

Prescribing Decisions
We prescribe only within our scope of practice and in our professional judgment. A visit does not guarantee a prescription. We do not prescribe on request alone, and we do not prescribe for performance enhancement. If medication is appropriate, we will review the options, benefits, and risks with you during the visit, and you may decline any recommended medication. The visit fee applies whether or not a prescription is issued.

Pharmacy Selection
Prescriptions are sent electronically to the pharmacy you designate. Once transmitted, a prescription cannot be edited or redirected. Please confirm the pharmacy’s hours, location, stock, and pricing before making your selection.

Refills and Changes
The visit fee covers the visit for which it was paid. It does not include future refills or medication changes, which require a new visit. As a single exception, within seven days of your original visit we may, at our discretion, approve one adjustment related to the same condition. After that period, or for a different condition, a new visit is required.

Refills are not automatic. We continue to prescribe only when the medication remains safe and effective for you and you are following your plan of care. We may modify or discontinue a medication that is not providing benefit or is causing serious side effects, and we may decline any prescription that we consider unsafe. Where clinically appropriate, we may limit the quantity or duration of a prescription.

Controlled Substances
Prescribing of controlled substances is subject to additional federal and state requirements, which may include identity verification, review of prescription monitoring program records, in-person evaluation, and separate agreements. We may decline to prescribe a controlled substance by telemedicine where it is not permitted by law or where we consider it clinically inappropriate.

Safeguarding Your Medication
You are responsible for the safekeeping of your medication. If your medication is lost, stolen, or destroyed, notify us immediately. We do not automatically replace medication, and each request is evaluated on a case-by-case basis.

Medication Side Effects
We prescribe with care; however, all medications carry a risk of side effects. Some are minor and resolve within a day or two, such as mild stomach upset. Others, such as constipation or changes in appetite, may persist, and identifying the appropriate medication and dose may require adjustments.

If you experience signs of an allergic reaction, discontinue the medication and seek immediate medical attention. For any other serious or concerning symptom, contact us immediately. If symptoms are severe or life-threatening, call 911 or go to the nearest emergency room.

Pharmacy Delays
Pharmacies occasionally delay a prescription because of fax processing, stock shortages, or the need to verify information with us. If this occurs, contact us by text or through the patient portal, and we will follow up with the pharmacy.

6. Telemedicine

Benefits of Telemedicine
Telemedicine eliminates travel and waiting-room time and allows scheduling flexibility, including evenings and weekends.

Limitations of Telemedicine
Compared with an in-person visit:
-
A hands-on physical examination is not possible, and some assessments may be less complete.
-
In-office testing, such as urinalysis, is not available.
-
Technical problems, including poor connectivity, may interrupt or delay the visit.
-
Telemedicine is not appropriate for emergencies.
-
Despite appropriate safeguards, no electronic system is immune from privacy or security incidents.
-
Certain medications may not be prescribed by telemedicine under applicable law.
If we determine that you require an in-person evaluation, we will direct you to the appropriate setting, including urgent care or emergency services where indicated. We may end or reschedule a telemedicine visit if the connection or setting prevents us from providing safe and effective care.

Your Responsibilities
For a telemedicine visit, you are responsible for the following:
-
Maintaining a stable internet connection, a working camera, and a microphone.
-
Participating from a private, quiet location, and not attending the visit while driving.
-
Being physically located, at the time of the visit, in a state where your provider is licensed to practice, and confirming your location and identity at the start of each visit.
-
Completing all pre-visit instructions and reporting any technical difficulty immediately.
By choosing a telemedicine visit, you acknowledge and accept these limitations.

7. Patient Expectations

Honesty and Trust
Effective care depends on honest communication and mutual respect. Please report your symptoms, side effects, and health concerns accurately and completely, including any difficulty following your treatment plan or taking your medication. If you misrepresent or withhold information in a manner that makes treatment unsafe, we will not issue a prescription and may end your care.

Resolving Disagreements
We recognize that you and your provider may sometimes see matters differently. To keep discussions productive, we ask that you:

-
Ask questions, including about information you have read, heard, or experienced previously.
-
Share your perspective and the reasons for it.
-
Refrain from demanding a specific medication or treatment. If we cannot agree on a plan of care, you may need to seek a different provider.
-
Not record any visit without our prior consent. Applicable law generally requires the consent of all parties to record a conversation. You may take notes or bring a support person.
-
Raise disagreements respectfully and, if they are not resolved during the visit, address them afterward rather than escalating them during the visit.
After a visit, you may email us your concerns and the resolution you are seeking. We will respond within one business day and advise you of what we are able to do.

Grounds for Declining or Discontinuing Care
We may decline or discontinue care for reasons including:
-
Dishonesty or omission of information that makes treatment unsafe
-
Persistent unwillingness to consider our recommendations
-
Failure to follow the agreed treatment plan
-
Repeatedly missing or rescheduling follow-up visits
-
Failure to complete recommended referrals or diagnostic studies
-
Signs of severe physical or mental decline that make outpatient care unsafe
-
Unusual behavior that raises safety concerns, such as agitation, irritability, or extreme drowsiness
-
Ongoing unsafe substance use
-
Excessive, abusive, or inappropriate messages, calls, or texts to our team
-
Threatening, aggressive, or intimidating behavior toward our team
This list is not exhaustive. Other circumstances may make it unsafe or unreasonable to continue treatment or prescribing. Where clinically appropriate, we will provide reasonable notice and assist with the transition of your care, as described in Section 1.

8. Informed Consent
